Change of circumstances form

This page explains when you should use the change of circumstances form for the points-based system, the information you should include, and where you should send the form.

When should you use the change of circumstances form?

You should use this form to notify us of any changes to your details while you are in the UK, before or after your application has been decided.

You do not need to tell us if your address changes, unless your application has not yet been decided.

You must complete the form in block capitals using a black pen. Enter all dates as dd-mm-yyyy (for example, 29-04-2000).

What information do you need to include?

When you complete the form, you should include the following information:

  • a description of the change;
  • your reference number for the points-based system; 
  • your representative's reference number (if appropriate); 
  • your date of birth; and
  • your nationality.
